What do you think about my letter requesting a leave of absence due to maternity? - qualified written request

I must be writing a letter to the birth of my child to apply for. I do not qualify for FMLA or short term due to the fact that I am not at my job for 1 years. If the check ... Is there anything I should add, or to return to the word?

I speak with your manager and human resources before they are sure in writing to you all on the same page. The letter is a formality for your records.

I think we should give time for the birth, which is 6 weeks of family medical reasons, you are not eligible to leave. My situation was that I moved to Tennessee from CA. Tennessee has no state disability as a CA. I, like you, was not on FMLA by my current employer. I told them if I on maternity leave, and they were very good. I was on disability and are eligible for FMLA CA, is paid for the CA. Talk to them and hope to understand them.
